SPECTRUM DATA SAVER:  This is an optional HIPAA Compliant Disaster Recovery Program.  Please select the level of coverage desired.  (First 30 days of coverage is at no charge)

NOTE: An Extended Service Agreement for a second system qualifies for a 50% rate reduction.

IMPORTANT:
A. Remittance must be submitted with this Agreement. This agreement will automatically renew with the same annual, semi-annual or quarterly schedule upon the anniversary date as applicable. Cancellation of the agreement by the client required 30-days advance written notice.
B. It is the customer’s responsibility to monitor the status of this Agreement.
C. ESA services will only be provided to participants who keep their account current. For accounts that are past due, services will be charged at prevailing rates until past due amounts are paid in full.
D. If the ESA has expired, a reinstatement fee is required as follows:  $250 is expired up to 45 days and $500 if expiration exceeds 46 days.

The undersigned acknowledge they have read the Extended Service Agreement Terms and Conditions attached. Further, the undersigned agree the ESA is the complete and exclusive statement of the agreement between the parties and supersedes all proposals or agreements, oral or written, and all other communications between the parties relating to the matter of the ESA.

This Agreement is not transferable to another customer or to another Axon System.
